DATA SUBJECT APPLICATION FORM

1. APPLICATION METHOD

Pursuant to the Law on the Protection of Personal Data No. 6698 (“KVKK”), individuals defined as data subjects (“Applicant”) are granted the rights listed under Article 11 of the Law. Accordingly, everyone may apply to the data controller and request, with respect to their personal data:

a) To learn whether personal data are processed,
b) To request information if personal data have been processed,
c) To learn the purpose of personal data processing and whether they are used in compliance with the purpose,
ç) To know the third parties to whom personal data are transferred domestically or abroad,
d) To request the rectification of incomplete or inaccurate personal data,
e) To request the erasure or destruction of personal data under the conditions set forth in Article 7,
f) To request that the actions taken pursuant to subparagraphs (d) and (e) be notified to third parties to whom personal data have been transferred,
g) To object to the occurrence of a result against the individual arising from the analysis of personal data exclusively through automated systems,
ğ) To request compensation for damages arising from the unlawful processing of personal data.

Pursuant to Article 13(1) of the Law, applications to be submitted to our Company, acting as the data controller, with respect to these rights must be submitted in writing or through other methods determined by the Personal Data Protection Board (“Board”).

Accordingly, written applications may be submitted to our Company by printing and completing this form and:

• Submitting it in person by the Applicant with proof of identity,
• Sending it via a notary public,
• Sending it to the Company’s registered electronic mail (KEP) address by signing it with a “secure electronic signature” as defined under the Electronic Signature Law No. 5070,
• Sending the completed application form via mobile signature or by using the e-mail address previously notified to the data controller and registered in the data controller’s system, to kvkk.map@most-amazing-places.com.

Other application methods to be determined by the Board shall be announced by our Company following their publication.

Applications duly submitted to our Company through the channels specified above shall be responded to within thirty (30) days from the date of receipt, pursuant to Article 13(2) of the Law. Responses shall be delivered in writing or via electronic means in accordance with the Law.

4. SUBJECT OF THE REQUEST

Please clearly state your request regarding your personal data and attach any explanations, suggestions, information, and supporting documents.

5. FINALIZATION OF THE APPLICATION

Our Company shall respond to your application within thirty (30) days at the latest, depending on the nature of the request and in accordance with the applicable legislation.

If you request that the response be provided in writing, no fee shall be charged for responses consisting of up to ten (10) pages. For responses exceeding ten (10) pages, a fee of 1 Turkish Lira per page may be charged.

If you request that the response be provided via a recording medium such as CD or USB flash drive, a fee not exceeding the cost of the recording medium may be charged.

You shall be responsible for any consequences arising from the information and documents provided being incomplete, outdated, inaccurate, or in the event of an unauthorized application.
